Becoming a New Patient






Thank you for choosing Dexter Family Practice. As part of becoming a new patient, we do require your previous medical records in advance to your first visit. All new patients will meet with Dr Challa Reddy, Medical Director for a “get established” visit. At that time, you and Dr Reddy will determine if Dexter Family Practice can meet your medical needs. After your initial meeting with Dr Reddy, you may choose any of the practitioners here at Dexter Family Practice for your care. If you need to be seen for acute/urgent care while waiting for your records, we will be more than happy to see you as a walk-in patient. Insurance copayments are required at the time of each visit. Dexter Family Practice is currently not accepting patients who are uninsured. Sebasticook Family Doctors and Penobscot Community Health Care are Federally Qualified Health Centers who offer healthcare at low cost or free.


All patients of Dexter Family Practice are expected to have annual physical exams and any appropriate screening tests. These exams and any testing will be discussed with your provider prior to ordering. Annual health maintenance is recommended not only by Medicare and MaineCare, but also by most commercial insurance companies. Dexter Family Practice is committed to providing high quality healthcare. Any new patients requesting pain/narcotic management will be asked to provide a urine drug screen as part of our efforts to determine if we can indeed meet your medical needs. Refusal of this urine screen is optional. However, acceptance into the practice at that point is up to the discretion of Dr Reddy.


We highly recommend that all family members join our practice in order for us to provide family oriented healthcare. Individual family member’s medical information remains highly confidential unless waived by that member.
